Perhaps you aren't aware of this, but in the U.S. nearly every health
insurance company requires patients to go to their GP first for EVERYTHING,
and the GP then makes the decision to refer or to treat themselves. It is not
the patient's fault if the GP refuses to refer, although the wary consumer
should go to another GP, which under many plans involves finding an
approved"
GP who is currently taking new patients, and then officially changing your
Primary Care Physician with your insurance company before you can see the new

It's a pain in the rear, but I've done it. A lot of people wouldn't realize
it's necessary, especially if they were brought up to "trust your doctor".
