You don't. I guess most macro virus are .DOT files themselves.
A DOC file can't hold a macro, a DOT file does, and rename
them to .DOC. Word opens them without warning.
They act normally as normal documents, so you will not notice it.
This is the only way a macro can 'travel' togther with a 'document'
Then you open the infected .DOT/DOC file, and then the macro
infects your NORMAL.DOT. So it seems all your docs are infected,
while only the NORMAL.DOT is changed. How about a backup of your NORMAL.DOT?
