I ran into this same problem some time ago, and I finally discovered that if 

have assigned an icon to the Project, as well as an icon to an individual 
Form, the Form icon always gets used when mimimizing the app. When the Form 
icon was removed, the icon displays as I watned.  Hope that helps...
