SMART originates on the drive itself.  Its a protocol where the system
(could be BIOS, some OS, or app) asks the drive how its doing.  Ordinary
BIOS or I/O operations that failed at the drive, but subsequently passed
after frobbing by the drive's onboard retrys, ECC, recalibrations, sector
remapping, etc operations are recorded. BIOS or the OS doesn't ordinarily
see the recovered soft failures a drive manages to handle internally.
