I don't think that it's possible to use extended memory without switching to 
protected mode first but I know that you can use int66 (or something - EMM 
interrupts) to access expanded memory.
Some programs are bigger than 640K and they don't use extended memory because 
they do not load all at the same time. These programs use overlays build into 
the EXE file - Borland C++ for example. ;)
