The take-home lesson here is that it is cheaper to refuse to fill it out
than it is to fill it out incorrectly.
However, if you fill it out incorrectly depending on how you do it they may
not notice and therefore may not come after you. But if you fail to fill it
out expect a visit.
This is simply one of a great many indicators that the government has
become too invasive in our lives.
