As you imply she's mentioned in precisely the same way everywhere..

 "Carole Chapman starts working for Apple"

One possibility is that an original author invented this so that she/he
could track folk who copy their work!

I've dipped into a few book indexes (indices?) and she's not mentioned
in any I found.

Someone must know?  :-)

(If she's genuine, then she's got her footnote in history!)
