I haven't checked whether they can be combined yet, so I want to kill 2
birds at once. Did you send 2 application forms or one? What quantity
did it/they show you were entitled to, and what did you write in the
quantity applied for box(es)?

AFAIK the options are free. Check with your tax accountant.
