  Always remember to properly initialise all members of a structure
  variable that has automatic storage duration, otherwise the values of
  members will vary each time that the function is called.

  The wasy way to do this in C++ is to write

     BITMAPINFOHEADER2 bmih = { 0 } ;

  This initialises the whole structure to zero (the first member is
  explicitly set to zero, and the remainder are zero-initialised).

  I'll leave it as an exercise for you to spot the one important member
  of `bmih' in your original code that you forgot to initialise.  (-:
