I have a problem with IPF: I can't get :i1 and :i2 to work like they should.

When I compile the file above (with IPFC 3.00.007, June 9 1995) the
index looks like this:
  index1-level-1
    index1-level-2-ref1
    index1-level-2-ref2
    index1-level-2-ref3
  index2-level-1

And not like I want it to look like:
  index1-level-1
    index1-level-2-ref1
    index1-level-2-ref2
    index1-level-2-ref3
  index2-level-1
    index2-level-2-ref1
    index2-level-2-ref2

I have tried many variations and it seems that only the first :1 gets
:i2 tags under it.

Am I doing something wrong, have I misunderstood the documentation, or
is my IPFC compiler buggy (the one from DevCon #8) ?
