I do it the primitive way. Scan the form and set up a full page graphics box. 
The trick is to turn on Text Flows Through Box. Then you can go into graphics 
mode and type on the form. First set up your margins and line height 
appropriate to the form, then use carriage returns and spaces to position to 
the places you want to fill in.
