Most big accidents seem to result from a cascade of mistakes. Recent issue of 
Av Week fully covers the failure of the computer driven instrumentation on 
Airbus 300 aircraft when bank angles exceed 45 degrees or rtae of change 
exceeds prespecified limits. Says FAA is looking to call for chnges.
