John C has given the technical info (although I didn't read it to check).

The quick explanation is that a DHCP Relay Agent (or a router configured to
forward bootp requests) is configured with a static IP address and with the
IP of the DHCP server.

When a client broadcasts for a DHCP server, the relay agent forwards the
request to the DHCP server(s). The DHCP server can tell what subnet the
request came from because the request is not a broadcast. It is from a
computer (the relay agent) with a particular IP address.
