The predecessor of the ITU, CCITT, and ITU itself (which is the same
organisation) is a UN-agency now which has existed for as long as there has
been automated international dialing. The very first roots even date back
to 1921 when there was no universal automated local dialing either.

The ITU-numbering-plan was not made with Fido-requirements in mind. ITU's
numbering system was made with a global-vision in place while the
North-American plan only looks at the local market. The attitude is
"We put a one as country-code without consulting anyone, and
whatever's behind the 1 is none of your business". That attitude is
limiting the possibilities for the rest of the world which has to go in
places to 3-digit country codes because the North-American market has
become so untransparant that any fundamental change there might make the
system collapse.
The North-American system is very unstructured and anomaly is piled upon
anomaly, they're constantly creating holes to fill-up other holes without
regard to the rest of the world ... and the Canadian companies are not
exactly pleased by this but they have not many options open.

Nope, because the ITU-numbering plan has no 1-12x-xxx-xxxx number for North
America. The whole range of 3- and 4-digit special-service numbers from the
ITU and starting with "1 would not create havoc in North America.
