Sure.
Here is what I posted last year, split into eight messages - the
form is actually that sent out in 1994, so please ignore the prices
as no longer valid - see eighth message for latest info.

The following is a copy of a form sent to me by my father (now 89)
with the note "I used to get this years ago, and found it very
useful & helpful & interesting".  Alas ! He does little genealogy
now.  I have sent my form off already.  If you are too late for
this year, write to your nearest agent and ask for a form for next
year.  Or use this anyway!
I have no connection with this venture, but it seems worthy of my support.
