It was a blanket permission. Even if you had used it as the basis
for an upgrade, you could donate all original disks and
documentation to a school. They could then register it and use it
as the basis for an upgrade, but if they did not register it,
they could use it.
As for your drive, it's unrepairable. If you ever have to use a 5 1/4,
buy one. You can get a new one for under $50, and a used one for
maybe $25 or less. You won't get a repairman to touch it for $50.
