In the case of CIgna; their formulary consists of 2 tiers.

Tier 2 contains all of the generics and *specific* brand-name drugs that
have no generics at this time. Many big-name drugs like Lipitor, Nasonex
and the like are NOT on the Tier 2 plan; which can be a big problem for
some folks.

Tier 3 contains all drugs legally prescribable.

If you have a 2-tier plan and your doctor presribes a drug not listed, you
foot the bill for the entire drug amount; unless your doctor can convince
Cigna to cover the drug or he chooses a drug that is covered in Tier 2.

When I had my surgery the pulmonologist prescribed Lovenox for 10 days; it
took a call to Cigna to get it approved as it wasn't even listed in Tier 3;
had it not been approved I was looking at a $380 bill.
