 There are checklists avaliable. Hopefully someone will post one for you to 
see. I am working on an ADD/ADHD/Bi-Polar file area on my BBS but it is FAR 
from ready.

 Educational accomodation plan. When a child is suspect of having problems 
(physical or mental) there is an evaluation process and then a placement. You 
can have a 504 written and the child not be considered special education 
(commonly referred to as SPED). Some school systems use checklists where the 
action to be taken is listed and they check it off;
 sits close to the teacher or blackboard
 has instructions repeated to him or her once again after the class has been  
informed
 a behavior management plan will be developed between home and school
 Things of that nature. Properly implemented (seldom done) they can be a
