You'd be surprised at what these people don't put on formularies -- and why.

I just found out that 200mg Cimetidine is on the Humana formulary, but the
they're the only ones --no 400, no 800. That means it would cost me $5 for
a 30 day supply. I have no idea what the 400mg or 800mg would go for, but
I'm thankful I don't have reflux.
