It can mean excessive, extreme or abnormal.  Granted, this is a tertiary
entry for the word, but it remains a connotative one.

The word's primary definition includes "pathology," so let's skip to the
secondary definition, "indicative of disease or caused by disease."  This
leads us to a perfectly ambivalent use of the word.  If I were to say "Klahn
is pathologically fat," it could mean that your gross obesity is caused by
disease or it could mean that you have an abnormal desire to stuff your fat
face.
