There's no structure about it.  "Hmm, there's a scene change that we
can split there, so lets add an ad break" "That means there's not much
time until the actual ad break" "You're right.  We'll ignore that ad
break and continue the show."
So when you right to build to an ad break... it's not coinciding with
the actual ad breaks here...
Plus they had to remove content from some shows to fit in ads.
Especially The Simpsons.
