I assume it's a SCSI - what other devices are on the same bus?  Maybe
there's an ID conflict.  Can you try booting from floppy with only the
CD-ROM on the SCSI connector?  Are there more than one device with
terminating resistors (most look like a 10-pin single in-line package)?
