Oddly that's not the layout on this modem.
There is one bank of 4 jumpers for IRQ and that's plain enough
but for COM I've got just three 2 pin jumpers.
One labeled 0, one labeled 1 and one labeled SEL.

I figure 0 and 1 represent the binary equivelent of COM 1 thru 4
with no jumpers being COM 1 and 0 and 1 jumpered being COM 4

SEL seems to both allow flashing the COM port amd IRQ
and select either PnP or manual jumper for COM and IRQ
