I finally got there after finding the real model number in the HTML file.
I also find when one jumpers the pins labeled SEL it'll flash.
No jumper it just tells you what it's set for, IRQ, COM port.
Yup it's the V.90 flash too.

I found one for Win 95, didn't run it using the ones you provided

Totally correct in all regards but the modem didn't show up on
diagnstics until I jumpered SEL and the flash took.
If I can set it to the COM anfd IRQ I want I might just slap it in the
H.P. machine set back to PnP.

 sorry I didn't figure it out and get there sooner or I'd have bought
