In addition to what George White wrote to you:

In OS/2 there are things that look like a physical COM port but are
actually something quite different. This includes VModem ports, CFOS (ISDN)
ports and COM ports redirected via LAN. If you try direct I/O on these,
you'll fail miserably. In addition direct I/O isn't exactly multi-tasking
friendly.
