Standard Microsystems Corp. (SMC) Ether Elite 16 -- Possibly Ultra
Elite?  Definitely an SMC Ethernet 16 model card.

Same as above but slightly different model.  If you have a copy of the
Micro House Technical Library you can search for these cards via their
model numbers (8013EWC and 8013EPC) or you can use the FCC ID lookup.
The cards should have an FCC ID printed on the back.
