There is no real definition for a government that is specifically
Christian.  The Jewish nation was defined by the Torah, but there
aren't really any governmental concepts communicated to the church, per
se.  "Christian nation" refers to a people and a culture who embrace
(or at least claim to embrace) Christian morality.

2> What day of the week should the Sabbath be celebrated?

If you're under the Jewish law, Saturday.  If you're a Christian,
you're not under the law, so there is no required Sabbath.  However,
taking one day in seven to rest just makes good sense, so I'd suggest
Sunday.

3> Must Christians follow the Ten Commandments as written in the
Jewish Bible (Exodus)?

Depends on what you mean by "must".  If you mean that in order to
remain a Christian you must obey them, no.  If you mean that as an
expression of devotion to God, you choose give the 10 commandments
their proper due, then there's room for discussion.

4> Please state the Commandment word referring to the taking of human
life.
Is it "Do Not Murder" or is it "Do Not Kill"?

I can't type in Hebrew, but it means "murder".

5> Although each of the following church groups consider themselves
'Christian', which of them would you consider 'Christian'?

Groups aren't Christian.  Individuals are Christians.  However, an
individual that embraces the whole theology of each of these (yes,
unless stated otherwise):

Roman Catholic
Greek Orthodox
Seventh Day Adventist
Seventh Day Baptist
Pentecostal - Depends
Mormon - no
Methodist
Shaker - Don't know
Quaker - Don't know
Amish
Menonite
Unitarian - No
Prebyterian - Depends
Christian Science - No
United Church of Christ - Don't know
Episcopal - Depends
Jehovah's Witnesses - No
Baptist
Old Apostolic Lutherin
Lutherin

I suppose one could put "Depends" on each of these, since they all have
their liberal heretics.  Whether one is Christian or not depends on
their faith in Christ alone as their savior, and not rejecting the
essential doctrines.

The essential requirements can be found in the foundational creeds
(Nicene, Constantinople, Apostles')
