I wouldn't say McCoy was sent to a critical focal point in history. He
went to whatever history the Guardian was playing at the moment. There
are any number of things he could have done in any time period to
unknowingly change history.
Kirk and Spock, on the other hand, had the Guardian replay the archival
footage and Spock, having recorded when McCoy jumped through on his
tricorder, and perhaps with a little help from the Guardian (he/it would
have nothing to gain from history being changed) and the theory of time
flow being consistent jumped through at the approximate time.
As far as the self-exiting/had been made dialogue that is just bad
writing or just one of those mistakes that are made in any film such as
Kirk giving out wrong stardates and planet names in the log in "Spock's
Brain."
