I recall reading a piece about the Apollo (might have been Gemini) program.
There were all sorts of bugs but the hardware ones were particularly
interesting. In once case, there was an event that didn't take place
within the specified number of milliseconds. The bug was marked as being
one that would not be fixed. Instead, the anomalous behaviour was fully
documented.
