Hi Landon,
  You assume correctly.  the 5 is the lug holes, the 4 3/4 is the diameter
of the circle that the 5 holes are placed on. (if you scribed an arc through
all the lug hole centers, the diameter of this circle is 4 3/4")
 "Modern" full size cars and Pick Ups have a 5 on 5 bolt circle.
