No, I wasn't disabling them. I didn't even have a command handler for them :)
(I believe that's why they got disabled). I removed the MN_ command ID's from
the two SUBMENU lines and now everything is working correctly.
I had looked at the examples countless times without noticing that the various
SUBMENU lines only had a string and not the command-identifier following .
