I don't have any separate program design documentation but I _rely_ on
the comments attached to the various modules about how they operate, and
the the use of appropriate (and sometimes quite long) variable names and
heavily commenting structs.
The worst job as far as I'm concerned is writing _user_ documentation,
most of them don't read it (they don't even read the essential things
that are put up on the screen during use - I had an example of that
yesterday on site). For commercial products they require it to be
written and provided, and then require training courses to tell them
what's in the documentation.
