OS/2 Warp v4 FP12, 9G Segate LVD disk. Using either System Commander v2.3
or normal DOS 6.22 boot.

OS/2 is probably not involved in this because the System Commander and DOS
boot disk what is hit first in my boot system.

Yes, I followed the directions to the letter. And I even tried to move
the controller to another slot to see if that mattered.
