Menus aren't too difficult.  If you can find a sample application
in a book and study the format of the menu resource, you should
be able to make your own.

Doesn't exist.  That's the biggest reason why I just upgraded from
GCC to Watcom.  (GREAT upgrade, BTW...faster compiler, smaller EXEs,
toolkit, good debugger).  I believe IBM sells the toolkit seperately,
but I don't know how much it costs.

You really need a dialog editor for that.... (unless you want to
spend days studying IBM's reference guide to resources...)
