I installed the same files you have. During the UDF format I got a trap 000e in
os2krnl. After rebooting and trying to access the drive via the drives icon I got
another kernel trap. I was able to access the drive with RSJ. My drive, a Liteon
LDW401S combo drive (uses dvd+rw media and any cd media) is not on IBM's list of
tested drives. Is it possible that my drive is not compatible or capable?
My system is Warp4/fp15 with kernel version 14.096e_W4.

Any suggestions?

Thanks,
Duncan
