those defaults are

     Login Attempts: 3
         Login Time: 30
    Password Change: 0
   Password Inquiry: Yes
  Password Attempts: 3
    Start Code Page: CP437
   Use Matrix Login: No
        Matrix Menu: matrix
    Matrix Password: matrix
         Matrix ACS: s50
Invisible Login ACS: s255
  See Invisible ACS: s255
    Multi Login ACS: s255

in your General Settings, you probably have an Inactivity timeout of 300 (5
minutes)... can you test if you change your inactivity timeout to the same
value as the login time and see if you get the warning? try 3 minutes for
both and then try 10 minutes for both... you already know what happens with 5
minutes ;)
