Is your program VIO or AVIO?  For VIO (full-screen) programs, I just
use 0 for the HVIO parameter.  For AVIO (VIO calls in a PM window), I
call VioCreatePS & VioAssociate in the WM_CREATE processing for my
main window, and then pass the resultant HVPS as the HVIO parameter. 
If you'd like to see a code fragment, just let me know....
