My Windows 95 .inf file lists this for a bunch of Hayes modems,
including the Optima 288 V.FC

Hayes 28800 Optima

Pick up the phone, dial a single digit, listen for noise.
Pops, crackles, or hums of any kind will create errors.

I made Mom's work better by disconnecting and reconnecting the
phone wires in the outside box. Helps clean off any corrosion.
Could also be a bad inside socket or plug, or just a noisy line.
Newer modems can handle noise a lot better.
