Did you watch carefully?  The error for the .EXE file may have scrolled by.

It means that the 1.11c patch wasn't successful.  I'd start over with a 
clean v1.11a and apply the "b" patch again...watch carefully for errors!

NONE of the above are needed once the patch process has completed 
SUCCESSFULLY (which is not the case in your situation)...once you get a 
"working V1.11b or 1.11c you can safely delete these

The only changes I saw for the "b" version were in a file called QM111B.NEW
