  Incidentally, the _only_ things that are necessary to do in
  _DLL_InitTerm/_dllmain/__dll_initialize are to save the module handle,
  and to initialise the C++ RTL.  Everything else is better done in the
  constructor and destructor of a class instance with static storage
  duration.
