It sounds like the common death of a defective heater where it fails to 
shut off.

And this sounds like the consequences of a heater that is insufficiently 
powerful to supply enough heat when the room temperature goes down.

That is a second possibility.  Many are adjustable and have a light that 
  is visible when heating occurs.  If it is necessary for the heater to 
be on constantly to achieve the desired temperature then there is no 
margin for severe room conditions.

Also:  If this is a large and elaborate tank then it would be prudent to 
have one heater set to do the brunt of the work and a second one set to 
turn on at perhaps one c degree  or 2 f degrees lower-- to pick up the 
slack during severe room temperature conditions and/or in the event of 
failure of heater#1
