In my experience, it's the modem that must do the caller ID.  I run USR
v.everthings here, with the current flash rom code.  In Binkley I've
changed the modem statements from "MODEMRINGING RING" to

;ModemRinging RING
ModemRinging Nmbr
ModemRinging Mesg

and the modem itself has caller id turned on and stored in NVRAM (#CID=1). 
(If you're running a USR I'll gladly send you my whole NVRAM settings, but
I won't waste the bandwidth here otherwise).

A typical log sample looks like this:

# 08 Jul 00:19:10 BINK Ring
# 08 Jul 00:19:11 BINK Date = 0708
# 08 Jul 00:19:11 BINK Time = 0119
# 08 Jul 00:19:11 BINK Nmbr = 2667754
# 08 Jul 00:19:14 BINK Ok

etc.   Results will vary slightly according to your telco's implementation of
the caller id display.
