I don't know if it applies to your situation(s), but there are buggy 
versions of the floppy driver that cause the floppy drive to stop working 
after APM has been active.  If it fails right after bootup, then this 
doesn't apply, but if it works at first, you will want to update to Device 
Driver FixPak 3 (I think that is what had the fix).
