The fast reply: you should study early American history.
The long reply: the US medical system does not deliver generally equal
treatment to all its citizens. An argument could easily be made that the
political will to maintain such a system places the responsibility for each
"less than equally cared for patient" death into the category of
'killing its own civilians.'
