Logitech wireless mouse, USB with a USB to PS2 adapter.
Stopped working in one machine (in PS2 port). New batteries didn't seem to
help. Didn't work in either primary or maint partition - primary uses
amouse; maint uses mouse.sys.
Worked on 2 other machines (USB port on one, PS2 through a KVM on the
other).
Corded mouse (again, USB through a green USB->PS2 adapter) worked OK.
Added usbmouse.sys to config.sys. Now everything works!
