Unfortunately, all the training in the world won't prepare any officer for 
this
type of situation.  Thats where a certain amount of common sense comes into
play, enough to reasonably guarantee the officers safety, enough to keep the
suspects attention and reasonably guarantee their safety.
Guess it all comes down to the amount of common sense, translated to reading
people from a distance to determine their threat level.
