1a.  Bart's middle name - Jojo - has never been mentioned on the show.
He got his middle name because of a conversation between Nancy
Cartwright and Matt Groening that went something like this:
NC: What does the J in Bartholomew J Simpson stand for?
MG: What do you think it should be?
NC: Jojo.
MG: Then Jojo it is.
(I think one of the books says it's Jebediah.)

Bonus trivia:

11. Janey's last name is Henderson on the TV series but Powell in the
merchandising.

12. Speaking of merchandising, in most sanctioned merchandising,
Bart's shirt is blue.  (Presumably, this is to make it easier for
Fox's legal department to find the knockoff stuff, which would have
Bart wearing a red/orange shirt.)

13. The teachers are not named for streets in Portland; Mrs. Krabappel
is named for Miss Crabtree from the Our Gang/Little Rascals comedies;
Miss Hoover is named for Mrs. Hoover, one of Matt Groening's teachers
(first grade, I think).  In fact, the first time she is mentioned on
the show, Lisa calls her Mrs. Hoover.
