One method is a simple loop trying to DosOpen() a file with
OPEN_FAIL_IF_EXISTS, starting at 00000000.000 and working upwards.

I see what you mean. You want guaranteed uniqueness, under all
circumstances.

Don't use hex then. Use a higher base. Including all letters of the
alphabet takes you up to base 36. In base 32, 6 bytes is less than 10
digits. Including all of the punctuation characters valid on SuperFAT
will let you use even higher bases. Remember that SuperFAT is
case-insensitive, though.
On the other hand, you could use long filenames (most of the non-FAT
filesystems on OS/2 will allow this) and have simple human-readable
filenames in the form :
1995Feb09.211158.17.1.1
Which is a lot easier to code. (-:
