  I'll need to check this out, I have the original Team Handbook by 
Joiner Assoc. (Copyright 1988--19th printing 5/93) and use it in 
teaching "teams" and "Quality Awareness" to my students.  I am sure 
their latter book only changes the terminology to be more friendly to 
educators as the original was focused on the business setting.  It is 
none-the-less, a wonderful resource.
