www.gravis.com has a diagram posted for a joystick port to MIDI breakout box. 
(Might take some digging. I know it's in a FAQ or some such.) I don't know if 
it will be plug compatible with Yamaha's boards, but...
It does call on a quad channel op amp, diodes, resistors, and a high speed, 
low loss opto-isolator. Mine works like a charm!
